Credit Repair & Referral Service Contract Agreement

This agreement is for the purpose of improving the client credit profile and helping client establish and or obtain new credit

The client agrees to:

• Provide a copy of all correspondence from credit bureaus or credits related to this case. Client Initials____________
• Provide clear copies of driver license number, social security number, and a utilities bill, failure to provide the above documentation's will be deemed as noncompliance with terms of this contract, and SPA will not accept responsibilities for any processing delays on this contract Client Initials_____
• Obtain all necessary and original credit reports and forward to SPA within 24 hours of receipt. Failure to comply will extend the contract deadline date that amount of time delayed by client. If credit reports are not received by SPA within 45 days then this contract is deemed breached and the SPA reserves the right to cease further action all money paid thus far is forfeited. • Not apply for any lines of credit enter escrow within written confirmation from SPA that all contracted items in this note have been amended or deleted Client Initials ________
• Authorize SPA to execute a limited power of attorney. This limited power of attorney will authorize SPA to sign any letters sent to credit reporting agencies, and to eliminate any delay in credit clearing process. Client initials______.

The client understands that:

• The agreement may be canceled if the client does not comply with the terms herein. Client Initials__________
• This contract and its services expire 12 months from the contract date above or upon completion of services needed. Client Initials_________
• This is a signed contract/note. Failure to regularly forward documentation for all three credit bureaus does not relive the client of their obligations, regardless of the circumstances, to pay the installment note in full. I understand that, except herein provided, my membership is absolutely non- cancelable. Should be client default they agree to pay all costs of collections including but limited to collection agency’s fees up to 50% of the unpaid balance court costs and reasonable attorney’s fees all of which may be paid or incurred by the holder of this note. • SPA accepts no responsibility for any real estate escrow delays, any cancellations or real estate transactions and or any credit request denials client initials_______
• SPA is to prepare all necessary correspondence in settlement or clarification of certain derogatory information that is obtained in client credit report ad furnished upon request valid reasons for disputes.
